Highmark Health is seeking a Nurse Practitioner to deliver primary care in our Pittsburgh primary care offices, including weekend and on-call coverage on a rotating schedule under a supervising physician.

You will assess patients, document medical histories, order tests, prescribe medications, educate patients and families, triage calls, and participate in quality improvement initiatives while adhering to HIPAA and organizational policies.

Qualifications

  • MSN from an approved program and NP licensure in state of practice.
  • National Board Certification for Nurse Practitioners.
  • Prescriptive authority and DEA registration within required timeframe.

Responsibilities

  • Provide primary care for non-acute or chronic conditions.
  • Record medical history, perform exams, and develop care plans.
  • Order diagnostics, interpret results, and document records accurately.
  • Prescribe medications and provide wound care as needed.
  • Educate patients and families on health and treatment plans.
  • Triage phone calls and coordinate with the care team.
  • Participate in quality improvement initiatives and regulatory compliance.
  • Occasional weekend and on-call coverage as required.

Job description

GENERAL OVERVIEW

This job is an integral part of the care delivery system working together to provide a healing environment, by placing our customers first. The incumbent provides primary healthcare services for non-acute or chronic health conditions in specific populations in accordance with applicable scope and standards of practice and with the policies, values, and mission of the organization within the CRNP scope of practice. Work is performed in a physician office or offices dedicated to Primary Care, weekend, and on-call coverage on a rotational basis, as required.


ESSENTIAL RESPONSIBILITIES


  • Provides appropriate patient care to include recording patient medical history and physical assessment, developing therapeutic care plan, ordering diagnostic tests, interpreting test results, providing accurate medical record documentation. (25%)

  • Communicates and coordinates patient care with patient, family, community health services and interdisciplinary team. Involves attending physician regarding plan of care and involves physician in clinical decision making of any issues that are outside the CRNP's knowledge base or scope of practice. (15%)

  • Provides monitoring and continuity of care and coordinates periodic evaluation by physicians as defined in the written agreement between the CRNP and the supervising physician(s). (10%)

  • Prepares written prescription orders for drugs and controlled substances, administers medication and injections, sutures wounds and provides wound care. (10%)

  • Educates patients and family members regarding health and preventive medicine, medications prescribed, treatment instructions and recommends community resources as appropriate. (10%)

  • Triages patient telephone calls, provides monitoring and continuity of care. (10%)

  • Participates, develops, and implements performance and/or quality improvement initiatives. Ensures compliance with all regulatory agencies. (10%)

  • May perform other duties such as manage outpatient concerns and any other duty specific to specialty. (10%)

  • Other duties as assigned or requested. (10%)


QUALIFICATIONS


  • Required: Master's degree (MSN) from the practicing State Board of Nursing Approved Master's Program National Board Certification for Nurse Practitioners through an accredited association Practicing-state licensure as a Nurse Practitioner.

  • Preferred: None.

  • Prescriptive Authority from the practicing state Within 60 days of hire.

  • Drug Enforcement Agency Licensure Within 60 days of hire.

  • CPR – American Heart Association.

  • Act 34 Criminal Background Clearance Certificate.

  • Act 33 Child Abuse Clearance Certificate.

  • Act 73 FBI Fingerprinting Criminal Background Clearance Certificate.
