About the Role

We seek an experienced Nurse Practitioner to deliver primary care to homebound and palliative patients in Burlington County, NJ. You will apply medical techniques under physician supervision, caring for a diverse patient population with chronic diseases.

Responsibilities:

1. Provide high-quality care to primary and palliative patients in Burlington County, NJ.
2. Assess patient health through examinations, medical histories, and care planning.
3. Communicate effectively with office staff and across clinical platforms.
4. Deliver patient-centered care to your panel of patients.
5. Attend quarterly medical staff meetings.
6. Develop diagnoses and treatment plans.
7. Build trust and consult with patients, families, and caregivers.
8. Order diagnostic tests as needed.
9. Prescribe medications and necessary interventions.

Qualifications:

• Board Certified with valid NP or PA certification in New Jersey.
• Strong clinical skills for assessment, diagnosis, and treatment.
• Positive demeanor and excellent people skills.
• Ability to work independently and problem-solve confidently.
• 1-2 years of primary care experience with geriatric patients; hospice experience preferred but not required.
