Job summary

Matrix Medical Network seeks a Nurse Practitioner or Physician Assistant (PRN) to perform comprehensive in-home health assessments for health plan members. In this field-based role, you’ll conduct histories, physical exams, and risk assessments, identify care gaps, and address social determinants that affect health outcomes.

You will document findings in EMR systems, educate patients and caregivers, and collaborate with physicians and health plans to improve care quality.

Qualifications

• Licensed NP or PA with Virginia credentials
• Experience performing in-home health assessments preferred
• Strong communication and documentation skills
