Job summary

Optimal Care in Michigan seeks a Nurse Practitioner or Physician Assistant to deliver primary and consultative care to geriatric and medically complex patients, often in their homes or care settings, with autonomy to focus on patient-centered care.

You will conduct comprehensive medical evaluations, develop treatment plans, prescribe medications, and coordinate with physicians and care teams to help patients and families understand diagnoses and avoid unnecessary hospitalizations.

Qualifications

• Master's or doctoral degree as a Board-Certified Nurse Practitioner or Physician Assistant
• Current State License as a Nurse Practitioner or Physician Assistant
• Reliable transportation, valid driver's license, and valid automobile insurance coverage
• Minimum 2 years clinical experience in a post-acute setting or with geriatric patients
• Adult, Family, or Gerontology certification

Responsibilities

About the Role

Practice medicine in a way that prioritizes time, connection, and meaningful patient relationships. As a Nurse Practitioner or Physician Assistant in Physician Services, you'll provide primary and consultative care to geriatric and medically complex patients, often in their homes or care settings, with the autonomy to focus on what matters most-delivering high-quality, patient-centered care that improves comfort, function, and quality of life. In this role, you'll conduct comprehensive medical evaluations, develop and manage individualized treatment plans, prescribe and monitor medications, and coordinate closely with physicians, caregivers, and interdisciplinary team members. Working within a collaborative practice framework, you'll serve as a key clinical decision-maker while helping patients and families better understand diagnoses, manage chronic illness, and avoid unnecessary hospitalizations.

Location

Office Location: Jackson, MI
Main Service Area: Lansing and surrounding areas
This role is a high travel position and requires daily travel within the service area to provide care to patients in their homes, assisted living facilities, or nursing homes

Hours

Monday to Friday, 8 a.m. to 5 p.m.

Required Qualification

Preferred Qualifications

Pay Range

This role provides a base annual salary (listed below) and has monthly bonus potential

$110,000 - $125,000 USD
