Millennium Physician Group in Florida is seeking an Outpatient Pulmonary Advanced Practice Provider. You will work with a supervising pulmonologist and multidisciplinary team to deliver comprehensive outpatient respiratory care in a value-based care setting.

The role emphasizes clinical autonomy, patient relationships, and collaboration with a strong care infrastructure to improve outcomes in Southwest Florida.

Qualifications

• Experience in pulmonology or outpatient internal medicine is preferred.
• Active Florida APRN or Physician Assistant license in good standing.
• National Board Certification as an APRN or PA.

Responsabilités

• Provide comprehensive outpatient pulmonary care in collaboration with supervising pulmonologist.
• Perform detailed respiratory assessments and diagnostic reviews.
• Evaluate and manage acute and chronic pulmonary conditions (COPD, asthma, pulmonary fibrosis, ILD, nodules).
• Interpret pulmonary diagnostics, PFTs, chest imaging, labs, sleep studies.
• Develop and modify treatment plans based on evidence-based guidelines.
• Educate patients on inhaler technique, medication adherence, oxygen therapy, smoking cessation, and self-management.
• Coordinate care with PCPs, specialists, and rehabilitation programs.
• Ensure compliant documentation and billing in the EMR.
• Collaborate with medical assistants and office staff to maintain clinic workflows.

Through proactive management of respiratory conditions and strong care coordination, you'll help prevent exacerbations, reduce hospitalizations, and improve patients' quality of life. ### **What You'll Do*** Provide comprehensive outpatient pulmonary care in collaboration with the supervising pulmonologist.* Perform detailed respiratory assessments, including patient histories, physical examinations, and diagnostic reviews.* Evaluate, diagnose, and manage acute and chronic pulmonary conditions, including: COPD Asthma Pulmonary fibrosis Interstitial lung disease Pulmonary nodules Sleep-related breathing disorders* Interpret pulmonary diagnostics, including pulmonary function tests (PFTs), chest imaging, laboratory results, and sleep studies.* Develop, implement, and modify individualized treatment plans using evidence-based guidelines.* Manage chronic lung disease with a focus on preventing exacerbations, emergency department visits, and hospital admissions.* Educate patients and caregivers on inhaler technique, medication adherence, oxygen therapy, smoking cessation, and disease self-management.* Coordinate care with primary care providers, specialists, pulmonary rehabilitation programs, home health services, and ancillary providers.* Identify care gaps and support quality initiatives, HCC documentation, and value-based care performance measures.* Ensure accurate, timely, and compliant documentation and billing within the electronic medical record.* Collaborate with medical assistants and office staff to maintain efficient clinic workflows.* Consult with the supervising physician regarding complex clinical cases and treatment decisions.### **Qualifications*** Active Florida APRN or Physician Assistant license in good standing.* National Board Certification as an APRN or PA.* Experience in pulmonology, outpatient internal medicine, respiratory care, or a related specialty preferred.* Strong knowledge of pulmonary disease management in an outpatient setting.* Experience interpreting pulmonary diagnostics and managing chronic respiratory conditions.* Excellent patient communication, education, and care coordination skills.* Commitment to evidence-based, patient-centered care.* Strong clinical judgment and ability to work collaboratively within a multidisciplinary environment.* Experience with AthenaOne EMR is a plus. ### **About Millennium Physician Group**Formed in 2008 and headquartered in Fort Myers, Florida, with offices throughout Florida, North Carolina, Georgia, and Texas, Millennium Physician Group is one of the largest independent physician groups in the United States.
