Job summary

Optum Home & Community Care is seeking a Nurse Practitioner to perform in-home assessments and manage care for patients in their homes across assigned Colorado areas. This role requires daily travel to patient homes and involves collaboration with an interdisciplinary team to coordinate preventive and ongoing care.

The position offers a $10,000 sign-on bonus, competitive compensation with base pay and bonus, and a comprehensive benefits package.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult‑Gerontology Primary Care.
• Active and unrestricted license in Colorado (or ability to obtain by start date).
• Active and unrestricted DEA licensure or ability to obtain post‑hire.
• Driver's license with access to reliable transportation.
• Ability to gain a collaborative practice agreement, if applicable.

Responsibilities

• Conduct comprehensive in-home assessments including medical history, meds reconciliation, vitals and exams.
• Manage acute medical situations or exacerbations and sub‑acute rehab episodes.
• Ensure accurate documentation including ICD-10 coding.
• Collaborate with interdisciplinary care team and provide patient/caregiver education.
• Engage in advanced illness discussions and care planning.

Education

ANCC or AANP Family/Adult/Geriatric certification

Colorado licensure (or able to obtain)

Active/Unrestricted DEA licensure

Driver's license and reliable transportation

The Optum at Home program is a longitudinal, integrated care delivery program that coordinates the delivery and provision of clinical care of patients (beneficiaries) in their place of residence. The APC within OAH provides care to our highest‑risk health plan beneficiaries and is part of an interdisciplinary team. Primary Responsibilities:

• Conduct comprehensive in-home assessments, including:
  • Past medical history review, medication reconciliation, vital signs, and physical exam
  • Evidence-based screenings and point-of-care testing (as appropriate)
• Manage members through an acute medical situation or an exacerbation of a chronic condition, including testing, diagnostics, interventions
• Manage members during a short term sub‑acute rehabilitation episode
• Ensure accurate and complete documentation, including ICD 10 conditions
• Communicate and collaborate with the interdisciplinary care team
• Conduct advanced illness and advanced care planning conversations
• Provide patients and caregivers with counseling and education

Required Qualifications:

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult‑Gerontology Primary Care
• Active, unrestricted licensure in Colorado (or ability to obtain by start date) without current disciplinary actions or practice limitations
• Active and unrestricted driver's license and access to reliable transportation
• Current, active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations (unless prohibited in state of practice)
• Ability to gain a collaborative practice agreement, if applicable in your state

Preferred Qualifications:

• 1+ years of clinical experience as an APC (family, geriatric, or home health preferred)
• Experience in geriatric medicine, long-term care, senior living or home care setting
• Experience in meeting the medical needs of patients with complex behavioral, social and/or functional needs
• Understanding of Geriatrics and Chronic Illness
• Understanding of Advanced Illness and end of life discussions
• Proficiency with electronic medical records and technology

Physical Requirements:

• Ability to navigate varied environments; ability to sit/stand/kneel as needed to perform duties

Compensation for this specialty generally ranges from $109,500 - $164,000. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. We comply with all minimum wage laws as applicable.
