Job description

This position is a hybrid opportunity in Riverside and requires the ability to work on-site with patient care (60%) and in case management (40%).

What You Will Do

Perform comprehensive health assessments including but not limited to the following:

• Obtain history and perform physical examination
• Complete review of medicines and related instructions
• Review home safety and psychosocial needs
• Fulfill documentation needs for STAR and HCC measures
• Obtain and document all pertinent information in patient's charts and case management notes on the same day the patient is seen; include medications, end-of-life preferences, and drivers of preventable hospital admissions
• Instruct all patients, caregivers, and family members to call prior to accessing medical care, subject to life-threatening emergencies
• Maintain patient safety and confidentiality
• Utilize universal precautions in all patient care
• Prescribe or order procedures, treatments, and medications within scope of practice
• Develop a high level of trust and intimacy with patients and families
