Sterling St James Inc. is seeking an experienced Advanced Registered Nurse Practitioner to join a patient-centered, community-based healthcare organization operating within the Patient Centered Medical Home model.

You will collaborate with physicians, ARNPs, PAs, and nursing staff to deliver high-quality, comprehensive care across a diverse patient population, while maintaining chart accuracy and contributing to quality improvement initiatives.

Qualifications

• Graduate of an accredited ARNP program.
• Current RN and ARNP licensure in the State of Washington.
• Prescriptive authority from Washington State.
• Experience practicing in a broad primary care clinic setting.
• Strong knowledge of the nursing process including assessment, planning, implementation, and evaluation.

Responsibilities

• Perform advanced clinical assessments and manage illnesses within ARNP scope.
• Collaborate with physicians, PAs, ARNPs, and nursing staff on patient care management.
• Develop and implement individualized teaching plans for patients' health.
• Use independent clinical judgment based on evidence-based data.
• Maintain chart documentation within 48 hours, 95% of the time.
• Complete fee slips with matching diagnosis and procedure codes.
• Meet productivity expectations (min 3,400 encounters per year).
• Provide care coordination through daily huddles as part of PCMH.
• Identify at-risk populations and monitor panel health.
• Educate patients for self-management and discharge planning.
• Explain procedures and treatment plans to patients before execution.
• Participate in quality management and staff education.

Position Overview

We are seeking an experienced Advanced Registered Nurse Practitioner to join a patient-centered, community-based healthcare organization committed to team-based care. This role operates within the Patient Centered Medical Home model and collaborates closely with physicians, ARNPs, PAs, and nursing staff to deliver high-quality, comprehensive care across a diverse patient population.

Key Responsibilities

• Perform advanced clinical assessments and manage illnesses and conditions within ARNP scope of practice
• Collaborate with physicians, PAs, ARNPs, and nursing staff on patient care management
• Develop and implement individualized teaching plans to support patients in achieving and maintaining optimal health
• Use sound, independent clinical judgment in patient care decisions based on evidence-based data
• Maintain timely and accurate chart documentation (within 48-hour timeline, 95% of the time)
• Complete fee slips with appropriately matching diagnosis and procedure codes
• Meet productivity expectations (minimum 3,400 encounters per year, full-time equivalent)
• Provide care coordination through daily huddles as part of the PCMH model
• Identify at-risk patient populations and monitor panel-specific population health
• Empower patients through self-management education and discharge planning
• Advocate for patients by explaining all procedures and treatment plans prior to execution
• Participate in quality management, peer review, and departmental practice development
• Support clinical education of other health professionals including student nurse practitioners
• Maintain all required annual training (HIPAA, Cultural Diversity, BBP, Sexual Harassment)

Qualifications

• Experience in minor surgical techniques (preferred)Strong knowledge of the nursing process including assessment, planning, implementation, and evaluation
