Nurse Practitioner NP Physician Assistant PA or Physician with a valid license to practice in the state of employment required License must be in good standingDemonstrated experience in providing patient care preferably in a home health clinic facility telehealth or specialty care settingStrong communication collaboration and interpersonal skills with the ability to work effectively within an interdisciplinary teamAbility to evaluate high risk patients develop care plans and coordinate care activities to meet patient needs and improve outcomesCommitment to delivering compassionate patient centered care and promoting a culture of excellence in healthcare deliveryStrong organizational and time management skills with the ability to multitask and prioritize responsibilities effectivelyAbility to read and communicate effectivelyStrong written and verbal skillsBasic computer knowledgeAbility to manage and demonstrate effective leadership skillsShould demonstrate good interpersonal and communication skills under all conditions and circumstancesAbility to foster a cooperative work environmentTeam player with ability to manage multiple responsibilities and demonstrate sound judgmentMust be able to work flexible hours and travel between offices facilities etc Must be a licensed driver with an automobile that is insured in accordance with state andor organizational requirements and is in good working order
