Gracelight Community Centers is seeking a Family Nurse Practitioner for a field-based Mobile Program. You will examine patients of all ages, treat acute illnesses and injuries, perform procedures, prescribe medications, and coordinate care across multiple locations.

Travel to sites is required; California NP licensure and DEA registration are essential. Strong EMR and Microsoft Office skills are expected, with a preference for bilingual SPANISH speakers.

Qualifications

• At least two years of experience working as a Nurse Practitioner.
• Advanced Practice Registered Nurse (APRN) licensure in California.
• Board-certified NP with active DEA registration.
• Experience with Patient Centered Medical Home model preferred.

Responsibilities

• Examine and treat acute illnesses, injuries and related conditions across ages.
• Perform procedures, write prescriptions, order tests within scope of practice.
• Provide wound care, incision and drainage when needed.

The Family Nurse Practitioner of the Mobile Program sees patients of all ages (0-99) to will examine and treat common acute illnesses, injuries, accidents and other injuries/illnesses; perform most medical services routinely handled within the scope of the advanced practice registered nurse’s practice; and perform selected laboratory testing, refer to specialists as needed, write prescriptions and perform office procedures within the scope of expertise protocols and available equipment. The Family Nurse Practitioner must be able to provide basic wound care, incision, and drainage, if needed.This is mainly a field-based role and requires travel. Must have a valid California Driver's License.
