Job summary

Conifer Park Outpatient Clinic in Schenectady, NY seeks a Nurse Practitioner to lead comprehensive medical history, exams, and medication-assisted treatment for substance use disorders. The NP will order tests, interpret results, provide care plans, and participate in team meetings and training.

Requirements include NY nursing license, NP certification, CPR, and DEA license; DATA 2000 waiver and Medicaid number are highly preferred.

Qualifications

• Post Graduate Degree – Master’s in Nursing.
• New York State Nursing License.
• National Certification as Nurse Practitioner.
• Greater than 1 year NP experience (preferred).
• Experience in addiction medicine or substance use disorder (preferred).
• CPR Certification.
• Unrestricted DEA Licensure.
• DATA 2000 waiver.
• Active Medicaid Number (Highly Preferred).

Responsibilities

• Perform comprehensive medical history and examination.
• Record initial diagnosis in the patient medical record.
• Order and interpret diagnostic tests (X‑ray, ECG, labs).
• Review test results and assess medication-assisted treatments for SUD.
• Provide induction, monitoring, titration, maintenance and tapering as appropriate.
• Evaluate emergencies and arrange appropriate referrals.
• Maintain naloxone and CPR readiness; educate patients on regimens.
• Participate in rounds, case conferences, and staff education.
• Prepare discharge instructions and follow-up plans.

Nurse Practitioner (Medical), Outpatient Clinic Sign on Bonus

Conifer Park Outpatient Clinic in Schenectady, NY is currently seeking a Nurse Practitioner. As a Nurse Practitioner with Conifer Park, you will be responsible for performing comprehensive medical history and examination, recording the initial diagnosis in the patient medical record, administering or ordering diagnostic tests (such as X‑ray, electrocardiogram, phlebotomy and other laboratory tests), reviewing and interpreting test results, assessing the appropriateness of medication‑assisted treatments for substance use disorders (including buprenorphine, methadone, naltrexone, acamprosate and disulfiram), providing induction, monitoring, titration, maintenance and tapering as clinically appropriate, evaluating and treating medical emergencies, providing emergency referral of unstable patients to appropriate facilities, maintaining training in the use of naloxone and emergency first‑aid response techniques such as CPR, instructing and counseling patients on compliance with prescribed therapeutic regimens, performing pre‑admission clearance and interviews when indicated, developing and implementing patient management plans, medication and therapy recommendations, assisting with continuity of care planning, performing and documenting follow‑up visits with updates in diagnosis and medications as needed, assessing and managing medication effects and side effects, participating in individual case assignments, case conferences, treatment team meetings, educational presentations for staff and patients, medical staff meetings, and additional duties as assigned by the Medical Director, providing medical on‑call coverage as assigned, participating in peer review activities and performance improvement initiatives, and preparing and educating patients for discharge medications and instructions.

Participates in individual case assignments, case conferences, multidisciplinary treatment team meetings, educational presentations for staff and patients, medical staff meetings, patient care monitoring conferences, and additional duties as assigned by the Medical Director.

Sign On Bonus: $7,000 SIGN‑ON BONUS (Based on 40 hours, will be prorated based on hours): $3,500 after completion of the orientation period, $3,500 after two (2) years of continuous employment.

Requirements

• National Certification as Nurse Practitioner (e.g., through American Nurses Credentialing Center, National Certification Center, or American Academy of Nurse Practitioners Certification Program)
