Circadia Health is seeking a full-time Nurse Practitioner to transform bedside care with AI. You will be on-site, see patients first, and coach facility teams to implement predictive health signals in real time across Louisiana facilities.

You will blend direct patient care with tech-to-bedside integration, train staff, and drive interventions while documenting care and collaborating with klinical ops and product teams.

Qualifications

• Must be a licensed NP in LA with ANCC or equivalent.
• Eligible for Medicare participation; company bills on your behalf.
• 2+ years NP experience with complex adult populations.
• Valid CA driver's license; comfortable traveling locally.

Responsibilities

• Direct patient care with initial and follow-up assessments.
• Train and coach facility staff to integrate Circadia into workflows.
• Interpret trend-based insights to drive timely interventions.
• Complete documentation within 24 hours of encounters.
• Perform high-volume rounds (roughly 25+ per day).
• Provide feedback to improve workflows and telemedicine/AI tools.
• Mentor NP onboarding and collaborate with clinical ops/product teams.

About the Role

Transform bedside care with AI. As Circadia’s on‑site NP, you’ll see patients first, act on predictive health signals in real time, and coach facility teams to turn data into earlier interventions—reducing preventable readmissions. You’ll blend direct patient care with tech‑to‑bedside integration through hands‑on training, live demos, and shadowed assessments.

This is a full‑time, in‑field position supporting our partner facilities across the state of Louisiana.

What You’ll Do

• Direct patient care: Conduct initial and follow‑up assessments on flagged residents; initiate/adjust plans of care; complete consent; coordinate with facility providers.
• Tech → bedside integration: Train and coach staff at the point of care, standardize use of Circadia into clinical workflows.
• Data into action: Interpret trend‑based insights, prioritize high‑risk patients, and drive timely interventions to prevent decline.
• Close the loop: Complete documentation/charts within the same day
• High‑volume rounding: Efficiently complete visits across designated SNFs (typ. 25+/day where appropriate), focusing on new admits, facility‑flagged patients, and Circadia alerts.
• Quality & product feedback: Surface field insights to improve workflows and tech integration in a clinical setting
• Mentor & collaborate: Support NP onboarding, share best practices, and partner with clinical ops/product teams

How Success is Measured (KPIs)

• Visits: Target 30 patient visits/day
• Engagement: ≥80% Time to Acknowledgement Rate: <24h
• Documentation: Complete all notes using AI‑powered tools within 24 hours of each encounter.
• Adoption & impact: Demonstrated workflow adherence at facilities; trend toward fewer falls/readmissions over time.

Must-Have Qualifications

• Certified and licensed Nurse Practitioner in LA; ANCC (or equivalent) in Adult/Family/Geriatric/Acute.
• Eligible for Medicare participation; company bills on your behalf.
• 2+ years NP experience with complex adult populations; able to practice independently.
• Valid CA driver’s license; comfortable traveling locally.
• Thrives in a dynamic, evolving startup environment.
• English required

• Spanish is preferred.

$130,000 - $160,000 a year

As a full‑time Nurse Practitioner, you will be employed by Circadia Health, Inc. The anticipated annual base salary range for this full‑time position is $130,000 - $160,000. The base range is determined by role and level, and placement within the range will depend on a number of job‑related factors, including but not limited to your skills, qualifications, experience, and location.
