Job summary

Montefiore Medical Center in New York seeks an experienced Nurse Practitioner to provide comprehensive care for cardiomyopathy and heart failure. You will lead patient care coordination, participate in quality initiatives, and mentor staff within a multidisciplinary team.

Qualifications

• Master's degree in Nursing required; DNP preferred.
• Current NY NP licensure and active DEA registration.
• National certification in adult-population NP; HF cert within 1 year.

Responsibilities

• Conduct comprehensive assessments, health histories, and exams via telemedicine or in person.
• Initiate and monitor guideline-directed medical therapy and advanced therapies.
• Order/interpret tests and coordinate echocardiography and labs per protocol.
• Oversee therapies across oral, subcutaneous, IV routes and in-clinic administration.
• Educate patients/families on plan, meds, interactions and side effects.
• Contribute to cardiomyopathy registry and data quality; support trials.
• Collaborate with pharmacists and access liaisons to ensure therapy access.
• Act as liaison for follow-up care across the care continuum.

Montefiore Medical Center is seeking an experienced Nurse Practitioner to provide comprehensive clinical care for individuals with cardiomyopathy and heart failure. This role combines direct patient care, interdisciplinary collaboration, quality improvement initiatives, and leadership within a highly respected healthcare environment.

As a key member of a dedicated multidisciplinary team, you will assess, diagnose, treat, and coordinate care.

Responsibilities

• Conducts comprehensive assessments, health histories, and physical examinations for an assigned panel, including interval visits after initial physician evaluation and post discharge visits, via telemedicine or in person.
• Initiates, titrates, and monitors guideline-directed medical therapy and advanced therapies under collaborative agreement and medical center protocols.
• Orders and interprets diagnostic tests within scope of practice, and coordinates protocol-driven echocardiographic and laboratory monitoring, including REMS program requirements.
• Oversees therapies across oral, subcutaneous, and intravenous routes, including in-clinic administration.
• Educates patients and families on treatment plans, medication use, interactions, and potential side effects.
• Assists with building and maintaining the cardiomyopathy registry, including patient identification, enrollment, and data quality.
• Participates in the evaluation, screening, and management of patients for investigational therapies and clinical trials, in partnership with the research team.
• Partners with clinical pharmacists and medication access liaisons to support therapy access and continuity.
• Serves as the liaison between specialists, patients, families, and care teams for follow-up care coordination across the care continuum.
• Maintains accurate documentation and ensures compliance with clinical standards and regulatory requirements.
• Engages in performance and quality improvement activities, and tracks the program's defined outcomes.
• Mentor staff and contribute to professional development and educational activities.

Requirements

• Master's degree in Nursing required; Doctor of Nursing Practice preferred.
• Current registration and New York State licensure as a Professional Nurse and Nurse Practitioner.
• Active DEA registration.
• National certification in an adult-population nurse practitioner specialty (e.g., AGACNP-BC, AGPCNP-BC, ANP-BC, FNP-BC).
• National certification in heart failure (e.g., AAHFN, HF-CERT) required within 1 year of hire.
• Prior outpatient heart failure experience strongly preferred.
• Experience with REMS-regulated or specialty therapies preferred.
• Experience with clinical research, patient registries, or investigational therapies preferred.
• Evidence of excellent interpersonal and communication skills, creative problem solving, excellent critical thinking, and leadership.
• Previously demonstrated ability to facilitate group processes and to work cohesively and collaboratively as a member of an interdisciplinary team.
• Qualified candidates must be able to communicate effectively with all levels of the organization.

Location & Work Schedule

• This position will support the Heart Failure and Cardiomyopathy Program across two primary locations:
  • Medical Arts Pavilion (MAP)
  • Hutchinson Metro Center (Hutch)
• The Nurse Practitioner is expected to work at both locations, with time generally split between the two sites based on clinical and program needs. Travel between locations will be required.
