Job summary

Woods Services in Langhorne, PA is seeking a Nurse Practitioner to provide primary care for children and adults served by Woods System of Care and affiliates, including individuals with developmental disabilities and brain injuries.

The role includes preventive and acute care, chronic disease management, health education, and care coordination within PA/NJ. On-site work with a commitment to documentation and team collaboration.

Qualifications

• BSN/MSN/DNP preferred and current NP licensure in PA/NJ.
• Minimum 3 years clinical NP experience, with vulnerable populations.
• Family practice NP certification and DEA/BLS credentials.
• Must meet state and federal clearance requirements.

Responsibilities

• Provide primary care to children and adults within Woods System of Care.
• Manage medications, labs, and chronic disease with care coordination.
• Document encounters in the EHR accurately and timely.
• Collaborate with Medical Director and IDT teams.
• Support billing documentation and external reporting as needed.
