Job summary

Wellpath is seeking an on-site Nurse Practitioner to provide primary and urgent care within a corrections facility in California. You will examine referred patients, run clinics, and respond to urgent situations while delivering Medication Assisted Treatment and chronic care management.

The role requires NP licensure in the state, CPR, and flexibility to rotate shifts. Experience in corrections or emergency settings is preferred and you will join a team focused on compassionate, standards-based

Qualifications

  • Graduate of an accredited school of nursing.
  • Advanced degree in an approved and accredited Nurse Practitioner program.
  • Experience in medical/surgical and/or emergency/trauma settings preferred.
  • Corrections experience preferred and able to work in a corrections environment.
  • Flexibility to rotate hours and respond to emergencies.

Responsibilities

  • Examine patients, conduct sick calls and rounds, respond to health emergencies.
  • Diagnose conditions, prescribe and manage treatment including MAT.
  • Conduct MAT assessments, inductions, follow-up, and titration.
  • Order/interpret labs and drug screens; utilize in-house and referral resources.
  • Document encounters, maintain confidentiality, and follow policies.

How you make a difference

The Nurse Practitioner provides on-site medical services by examining referred patients, conducting clinics, rounds, and responding to urgent and emergency situations. The role delivers ongoing medical care by diagnosing conditions, prescribing treatment plans, managing chronic care, and providing Medication Assisted Treatment services, including induction, follow-up, and medication management. The position documents care, orders and interprets labs, adheres to established policies and formularies, and supports continuity and standards of care within the facility.

Key Responsibilities

  • Examine patients, conduct sick call, chronic care clinics, and infirmary rounds, and respond to health emergencies.
  • Diagnose conditions, prescribe and manage treatment plans, and provide ongoing medical and MAT-related care.
  • Conduct MAT assessments, inductions, follow-up appointments, and manage medication continuation and titration.
  • Order and interpret laboratory tests and drug screens and utilize appropriate in-house and referral resources.
  • Document patient encounters, maintain confidentiality, and comply with established policies, procedures, and standards of care.

Additional Details

Candidates should have experience providing independent primary and urgent care, including patient assessments, diagnosis, prescribing, chronic disease management, sick call, and emergency response.

They should also be experienced in substance use disorder treatment and Medication-Assisted Treatment, including MAT evaluations, medication induction, continuation, dosage titration, follow-up care, withdrawal management, and relapse-prevention support.

Experience ordering and interpreting labs and drug screens, coordinating referrals, maintaining accurate clinical documentation, and working within established formularies and treatment protocols is important.

Correctional healthcare, reentry-focused care, Medi-Cal/CalAIM programs, and collaboration with multidisciplinary discharge-planning teams are strongly preferred.

Qualifications & Requirements

Education

  • A graduate of an accredited school of nursing
  • An advanced degree in an approved and accredited Nurse Practitioner program.

Experience

  • Prior medical/surgical and/or emergency/trauma experience and corrections experience is preferred.
  • Must have the patience and tact to deal with the inmate population and ability to work effectively in the corrections environment.
  • Scheduling flexibility is also required to be able to rotate hours and shifts, if needed, and to be called during emergency situations to provide coverage.

Licenses/Certifications

  • Must have and maintain current licensure as a Nurse Practitioner within the state of employment.
  • Must have and maintain current CPR certification.
