Job summary

Christus VNA Hospice, part of the LHC Group family, seeks a Hospice Nurse Practitioner to provide palliative and supportive care to hospice patients. The NP will consult with the Hospice Medical Director, coordinate care, and guide patient and family care plans.

Responsibilities include conducting face-to-face visits, collaborating with physicians, and maintaining detailed clinical documentation. A valid NP scope and state licensure are required for this field-based role.

Qualifications

• Active unrestricted license in state of residence.
• Current certification as a clinical Nurse Specialist or Nurse Practitioner (state of practice).
• Current national certification (ANCC/NBCHPN/AANP).
• 5+ years nursing experience.
• Current driver's license and vehicle access.

Responsibilities

• Make face-to-face encounter visits within prescribed time limits for Hospice patients.
• Consult with Hospice Medical Director and coordinate with primary physician.
• Initiate and coordinate the plan of care; document goals and patient/family response.
• Instruct and supervise patient/family in self-care techniques when appropriate.
• Maintain accurate clinical notes and report patient status to Administrator and Hospice Medical Director.

As the Hospice Nurse Practitioner, you will provide consultation in palliative care, symptom management and supportive care to meet the needs of the Hospice patient as requested by the referring physician.

Primary Responsibilities:

• Makes face-to-face encounter visits within prescribed time limits to qualifying Hospice patients and reports clinical findings to the Hospice Medical Director
• Consults with the Hospice Medical Director as needed, informs primary physician of recommended services, and collaborates with other physicians as needed
• Initiates and coordinates the plan of care. Documents problems, appropriate goals, interventions, and patient/family response to hospice care. Collaborates with the patient/family, attending physician and other members of the IDG in providing patient and family care
• Instructs and supervises the patient/family in self-care techniques when appropriate
• Maintains accurate and relevant clinical notes regarding the patient's condition. Reports patient condition and any identified needs/issues to Administrator and Hospice Medical Director

Required Qualifications:

• Active unrestricted license in the state of residence
• Current certification as a clinical Nurse Specialist or Nurse Practitioner in the state of practice
• Current certification by a national body such as ANCC, NBCHPN, or AANP
• 5+ years nursing experience
• Current driver's license and vehicle insurance, access to a dependable vehicle or public transportation

Preferred Qualifications:

• Current CPR certification or ability to complete within 90 days of hire
• Experience working with an interdisciplinary team
• Clinical nursing care in oncology, hospice, geriatrics, or intensive care
• Palliative Care ARNP

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $46.00 to $69.00 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
