Job summary

Medi Home Hospice, a division of Medical Services of America, Inc., seeks an experienced Part-Time/PRN Nurse Practitioner for hospice patients in State College, PA. You will provide high-quality care in collaboration with the Hospice Medical Director, focusing on health promotion, chronic disease management, and end-of-life care.

Responsibilities include face-to-face eligibility assessments, determining level of care, and maintaining patient records to support the interdisciplinary team.

Qualifications

• Master’s degree in nursing is required or preferred for NP licensure.
• Valid Nurse Practitioner’s license in PA is required.
• Previous hospice and palliative care experience is preferred.
• Strong oral and written communication skills are essential.

Responsibilities

• Perform face-to-face encounters to determine hospice eligibility within regulatory timeframes.
• Evaluate hospice patients for appropriate level of care.
• Assist in development of hospice protocols, policies and procedures.
• Prepare and maintain accurate patient records to support interdisciplinary team services.

Medi Home Hospice, a division of Medical Services of America, Inc., currently seeks an experienced Part-Time/PRN Nurse Practitioner for our Hospice patients in State College (Centre, Blair, Clearfield), PA.

Provides health care to patients and education to their families through the application of advanced knowledge and skills. In collaboration with the Hospice Medical Director, the Nurse Practitioner provides services related to health promotion, chronic disease management, and end of life care.

• Performs face-to-face encounters with the patient to determine continued eligibility for hospice care within time frame of regulatory guidelines.
• Evaluates hospice patients for appropriate level of care.
• Assists in development of hospice protocols, policies and procedures as required.
• Prepares and maintains accurate patient records and documents to support interdisciplinary team and services provided.

Qualifications and Skills

• Previous hospice and palliative care experience preferred.
• Excellent oral and written communication skills.
• Master’s degree in nursing.
• Valid Nurse Practitioner’s license in the state of PA.
