Job summary

Healing Partners is seeking a Nurse Practitioner for home-based wound care across private residences and community settings. The NP will lead clinical care, assess wounds, and coordinate post-acute transitions to support healing and prevent readmissions.

Must have NP credentials, RN license, and DEA registration. Daily travel within the assigned market is required; strong communication and collaboration with care teams are essential.

Qualifications

  • Successful completion of an accredited Nurse Practitioner program.
  • Active, unrestricted Nurse Practitioner/APRN license in the state(s) of practice.
  • Current Registered Nurse licensure in the state(s) of practice.
  • DEA registration and prescriptive authority as required by state regulations.
  • National board certification as a Nurse Practitioner.
  • Current BLS certification.

Responsibilities

  • Perform in-home wound assessments and develop evidence-based treatment plans.
  • Coordinate care with primary care, specialists, home health, and caregivers.
  • Deliver advanced wound care procedures in patient homes.
  • Monitor wound healing and adjust treatment plans as needed.
  • Ensure timely post-discharge assessments and treatment initiation visits.
  • Promote patient education on wound care and self-management.
  • Document all care activities accurately in EMR.

Nurse Practitioner – Home Visits Job Responsibilities:

The Nurse Practitioner (NP), Home-Based Wound Care, provides advanced clinical care to patients transitioning from Skilled Nursing Facilities (SNFs) to the home setting. As part of a transitional care model, the NP serves as a key clinical leader responsible for ensuring continuity of care, preventing avoidable complications, and supporting successful recovery following discharge from post-acute care.

The NP conducts in-home assessments, develops and manages evidence-based wound care treatment plans, performs wound-related procedures, coordinates care across interdisciplinary teams, and collaborates with primary care providers, specialists, home health agencies, caregivers, and referral partners. The provider is accountable for delivering high-quality wound care services that improve healing outcomes, reduce rehospitalizations, and support patients through completion of their wound care episode.

The position requires frequent travel within an assigned market to conduct patient visits in private residences, assisted living communities, and other community-based care settings.

Essential Functions
Clinical Care Delivery
  • Conduct comprehensive in-home assessments for patients transitioning from Skilled Nursing Facilities to the home.
  • Perform initial evaluations, follow-up visits, transitional care assessments, and wound care consultations.
  • Develop, implement, and manage individualized wound care treatment plans utilizing evidence-based practices.
  • Assess and monitor wound healing progression and modify treatment plans as clinically indicated.
  • Perform wound debridement utilizing sharp, enzymatic, autolytic, or other approved methods within the provider's scope of practice.
  • Perform associated wound care procedures safely within the home setting.
  • Apply and manage advanced wound care therapies, dressings, biologics, and wound management products.
  • Assess patients for risks associated with skin breakdown, wound recurrence, and infection.
  • Conduct diabetic foot assessments and ulcer prevention evaluations consistent with current clinical guidelines.
Transitional Care Management
  • Support safe transitions from Skilled Nursing Facilities to home-based care.
  • Complete timely post-discharge assessments and treatment initiation visits.
  • Identify barriers to care adherence and collaborate with Patient Navigators to improve engagement.
  • Monitor patients throughout the wound care episode to ensure continuity and treatment compliance.
  • Coordinate care plans with caregivers and family members to support treatment success.
  • Promote patient education focused on wound healing, disease management, prevention, and self-care strategies.
Care Coordination
  • Collaborate with Patient Navigators, Care Coordinators, and Operations teams to ensure seamless patient experiences.
  • Partner with primary care providers, specialists, home health agencies, therapists, and community resources to support comprehensive care.
  • Order, interpret, and act upon laboratory, diagnostic, vascular, and radiologic testing as appropriate.
  • Recommend referrals and ancillary services necessary to advance wound healing and overall health outcomes.
  • Facilitate procurement of wound care supplies, Durable Medical Equipment (DME), and related therapies.
Documentation & Compliance
  • Complete accurate and timely documentation within the organization's electronic medical record system.
  • Maintain compliance with state practice requirements, collaborative agreements, organizational policies, and clinical protocols.
  • Ensure all clinical documentation supports quality outcomes and regulatory standards.
  • Participate in quality improvement initiatives and clinical performance reviews.
Clinical Outcomes & Quality
  • Support achievement of wound healing and episode completion goals.
  • Assist in reducing avoidable emergency department visits and hospital readmissions.
  • Promote evidence-based clinical practices across all patient encounters.
  • Participate in case reviews, peer discussions, and outcome improvement initiatives.
  • Serve as a clinical resource for patients, caregivers, referral partners, and interdisciplinary team members.
Nurse Practitioner – Home Visits Qualifications and Requirements:

Education & Experience
Required
  • Successful completion of an accredited Nurse Practitioner program.
  • Master of Science in Nursing (MSN), Doctor of Nursing Practice (DNP), or equivalent advanced nursing degree.
  • Active, unrestricted Nurse Practitioner/APRN license in the state(s) of practice.
  • Current Registered Nurse licensure in the state(s) of practice.
  • DEA registration and prescriptive authority as required by state regulations.
  • National board certification as a Nurse Practitioner.
  • Current BLS certification.
Preferred
  • Wound Care Certification (WCC), Certified Wound Specialist (CWS), or equivalent.
  • Experience in wound care, post-acute care, skilled nursing, home health, hospice, palliative care, or transitional care management.
  • Experience providing care in patient homes or community-based settings.
  • Multi-state licensure.
Physical Requirements:
  • Ability to travel daily within assigned geographic territory.
  • Ability to safely enter patient residences and community-based care settings.
  • Ability to stand, walk, bend, kneel, squat, and lift as required to perform patient assessments and procedures.
  • Ability to lift and carry up to 25 pounds of medical equipment and supplies.
  • Fine motor skills necessary to perform wound care procedures and operate medical equipment.
  • Ability to drive and maintain a valid driver's license and acceptable driving record.
Work Environment:
  • Patients' homes, independent living communities, assisted living facilities, and other community-based settings.
  • Frequent local travel required.
  • Exposure to environmental conditions common to home-based healthcare delivery.
  • Potential exposure to bloodborne pathogens and infectious diseases requiring adherence to standard precautions.

Salary: $100,000.00 - $120,000.00
