Daymark Health is seeking a Nurse Practitioner/Physician Assistant to deliver hands-on, compassionate patient care in patients' homes and via virtual platforms. You will help implement a new oncology care model, triage high-acuity cases, and collaborate with a clinical operations team to support the full care continuum and improve patient outcomes.
The role requires 4+ years of direct clinical care, oncology/home care experience, and the ability to work full-time in-person in Orange County.

ABOUT THE ROLE
As one of Daymark’s Nurse Practitioners, you’ll work with our care team to implement our cancer care model with patients in their homes. Your primary goal is to provide hands‑on, compassionate patient care directly in patients’ homes and through virtual platforms. You'll also implement and help to continuously improve processes that will enable Daymark to be effective in Orange County, build a highly engaged patient population, and contribute to a high‑performing clinical team. You must be comfortable operating as a player/coach, rolling up your sleeves, and doing whatever is required to provide high‑quality clinical care and to grow quickly. You’ll work closely with our clinical operations team, leveraging your expertise to establish our evidence‑based oncology care and navigation model to address clinical (symptom management, in‑home urgent care, palliative care, mental and behavioral health care) and non‑clinical (social and related needs).

The expected compensation for this role is $140,000 to $170,000 annually, depending on location & experience.
This is a full time role operating on a 5 day / 8 hour schedule, Mon - Fri.
All Daymark Health patient‑facing roles require Basic Life Support (BLS) certification and compliance with Daymark's Employee Vaccination and Testing policy. For roles that require in‑home visits to patients, candidates must have a reliable personal vehicle and provide proof of a valid driver's license.
