Overview

Seeking Nurse Practitioners with 1+ year of experience to conduct In-Home Health Risk Assessments.

Responsibilities

• Conduct comprehensive Health Risk Assessments for Medicare Advantage patients in home settings.
• Review medical history, assess social environment, and identify care gaps to enhance care quality in a single visit.

Qualifications

• Board-certified Nurse Practitioner licensed in Virginia
• At least 1 year of NP experience
• Home Health experience

Compensation and Schedule

• Engagement: 1099 Independent Contractor
• Per assessment: $110 for each completed in-home Health Risk Assessment
• Assessment duration: 45 minutes – 1 hour
• Daily stipend: $60
• No-show payment: $25

Location and Hours

• Location: Choose counties to cover; extensive patient volume across the state
• Hours: Flexible terms; 7 days a week between 8am–8pm
