Job summary

Porter Cares is expanding into Hampshire, Hampden, and Franklin Counties and seeks Nurse Practitioners for in-home assessments and care management. You will evaluate medical history, review systems, perform vitals and a comprehensive exam to support tailored treatment plans.

Qualified candidates hold a Massachusetts NP license, with Medicaid experience highly preferred and at least 3 years of patient care. We offer flexible schedules, paid mileage, training, and iPad/equipment provided.

Qualifications

• Valid unencumbered Massachusetts NP license.
• Experience with Medicaid (highly preferred).
• 3 years of patient care experience required (primary care/adult/geriatric; EMR skills preferred).
• A strong sense of empathy, compassion, and friendliness.
• Exceptional communication skills, written and oral, with the ability to assist others with sincerity and respect.

Responsibilities

• Visit members within geographic area and conduct in-home health risk assessments including medical history review, systems review, medication reconciliation, vital signs, and comprehensive physical exam.
• Perform point-of-care testing as appropriate and identify diagnoses for care management.
• Collaborate with Porter Care Guides for active medical management and care planning.

What You'll Do

You will visit members within your geographic area of responsibility and conduct an in-home health risk assessment including an evaluation of past medical history, review of systems, medication reconciliation, vital signs, and a comprehensive physical exam. This evidence-based practice screening will include point-of-care testing (as appropriate), the identification of diagnoses to be used in care management by our Porter Care Guides and active medical management in the furtherance of treatment, and the formulation of a list of current and past medical conditions using clinical knowledge and judgment and the findings of your assessment.
