Job summary

Optum Home & Community Care, part of UnitedHealth Group, seeks an Advanced Practice Clinician (NP/PA) to provide care in nursing homes, senior housing and assisted living settings. No on-call, weekends, or holidays required; a generous sign-on bonus is offered for external candidates.

The Senior Community Care program coordinates care for high-risk members with bonuses for performance, and a supportive environment to grow professionally in geriatric medicine and chronic illness management.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geronticcare, or Adult-Gerontology Primary Care.
• Unrestricted state licensure with ability to start by date of employment.
• Driver’s license and reliable transportation.
• Ability to establish a collaborative practice agreement where required.

Responsibilities

• Perform comprehensive past medical history reviews, medication reconciliation, vital signs and exams.
• Conduct evidence-based screenings and point-of-care testing as appropriate.
• Maintain accurate documentation including ICD-10 codes.
• Collaborate with interdisciplinary care team to coordinate patient care.
• Lead advanced illness and end-of-life discussions and provide caregiver education.

The Senior Community Care (SCC) program is an integrated care delivery program that coordinates the delivery and provision of clinical care of members. The Advanced Practice Clinician within SCC provides care to our highest-risk health plan members. Primary Responsibilities:

• Past medical history review, medication reconciliation, vital signs, and physical exam
• Evidence-based screenings and point-of-care testing (as appropriate)
• Ensure accurate and complete documentation, including ICD 10 conditions
• Communicate and collaborate with the interdisciplinary care team
• Conduct advanced illness and advanced care planning conversations
• Provide patients and caregivers with counseling and education

Required Qualifications:

• For NPs: Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
• For PAs: Current NCCPA certification and state licensure
• Active, unrestricted licensure in the state you will be practicing in (or ability to obtain by start date) without current disciplinary actions or practice limitations
• Active and unrestricted driver’s license
  • In addition to a driver’s license, access to reliable transportation
• Ability to gain a collaborative practice agreement, if applicable in your state
• Ability to navigate varied environments and to position oneself as needed to perform job duties

Preferred Qualifications:

• 1+ years of clinical experience as an APC (long-term care setting preferred)
• Experience in geriatric medicine, long-term care, senior living or home care setting
• Experience in meeting the medical needs of patients with complex behavioral, social and/or functional needs
• Understanding of Geriatrics and Chronic Illness
• Understanding of Advanced Illness and end of life discussions
• Proficiency with electronic medical records and technology

Compensation for this specialty generally ranges from $109,500 - $164,000. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. We comply with all minimum wage laws as applicable.
