Job summary

Porter is expanding into Michigan and is seeking Nurse Practitioners for in-home care across Grand Rapids and throughout the state. You will conduct comprehensive health risk assessments, review medical histories, and perform examinations to guide care management with Porter Care Guides.

Ideal candidates hold a valid Michigan NP license and have several years of patient care experience with EMR proficiency, coupled with strong empathy and communication skills.

Qualifications

• Valid, unencumbered Michigan NP license.
• 3 years of patient care experience in primary/geriatric care.
• Experience with electronic medical records (EMR).

Responsibilities

• Visit members in their home and perform health risk assessments.

New team members will join feeling energetic and excited to revolutionize the healthcare continuum and contribute to the success of our mission-driven organization.**What You'll Do**You will visit members within your geographic area of responsibility and conduct an in-home health risk assessment including an evaluation of past medical history, review of systems, medication reconciliation, vital signs, and a comprehensive physical exam.
