Job summary

WellBe Senior Medical is hiring a Nurse Practitioner to deliver home-based primary care across the Glens Falls, NY area. You will conduct comprehensive visits, develop chronic disease plans, and coordinate with an interdisciplinary team to ensure safe transitions of care.

You’ll work with autonomy to prevent hospitalizations, serve high-risk seniors, and uphold our value-based care model with strong collaboration and patient-centered outcomes.

Qualifications

• Master’s degree in NP with focus on Adult, Geriatric, Acute Care and/or Family.
• Active NP license and board certification (ANCC or AANP).
• Driver’s license and access to a vehicle for local field travel.

Responsibilities

• Deliver field-based care: conduct comprehensive primary care visits and exams in patients' homes.
• Manage chronic disease with longitudinal care plans for high-risk patients.
• Collaborate with an interdisciplinary team to ensure safe care transitions.
• Prevent ER visits by treating acute needs in the home and keeping seniors safe.

Job Requirements

What You Bring:

• Advanced Degree: Master’s Degree (NP) with a preferred focus on Adult, Geriatric, Acute Care and/or Family.
• State Required Credentials: Active, unencumbered NP license and Board Certification (ANCC or AANP).
• Reliable Mobility: Valid driver’s license and vehicle for local field-based travel.
