Job summary

Rochester Regional Health is seeking a Nurse Practitioner, focused on Geriatrics, to deliver comprehensive primary and acute care for older adults across Long-Term Care and Assisted Living settings. You will work with a collaborative team across Monroe County and provide longitudinal care to medically complex geriatric populations.

This full-time role emphasizes patient-centered care, interdisciplinary collaboration, and strong onboarding.

Qualifications

• MS in Nursing is required.
• NP certification from NYSED is required.
• RN license with NYSED registration is required.

Responsibilities

• Provide primary and acute care for geriatric patients, including assessment and management of acute and chronic conditions.
• Care for Long-Term Care residents 2–3 days per week, including ventilator care collaboration with respiratory therapy.
• Care for Assisted Living residents 2–3 days per week in coordination with facility staff and Geriatric Medical Director.
• Coordinate care with nursing, respiratory therapy, physicians and other care team members.
• Maintain accurate medical records and monitor patient responses to treatment.
• Perform comprehensive physical examinations and develop treatment plans.

SUMMARYAs a Nurse Practitioner in Geriatrics, you’ll provide comprehensive primary and acute care to older adults across both Long-Term Care and Assisted Living settings. This role offers the opportunity to build meaningful relationships with patients while managing a diverse range of acute and chronic conditions. Provide care 2–3 days per week at Unity Living Center, including management of long-term care residents and patients requiring long-term ventilator support in collaboration with respiratory therapyAssisted Living. Provide primary care 2–3 days per week to residents in assisted living settings, working closely with facility staff and the Geriatric Medical DirectorCare Coordination. Collaborate with nursing, respiratory therapy, facility staff, physicians, and other members of the care team to coordinate comprehensive patient careDocumentation.
