Job Description
General Responsibilities
- Conduct thorough medical assessments to inform patient care.
- Design and execute tailored care plans for individual patients.
- Evaluate and treat acute illnesses in various settings.
- Engage actively in discharge planning to ensure continuity of care.
- Perform home visits as needed for patient assessment and support.
- Facilitate seamless coordination of care across the full spectrum of services provided.
- Contribute to the development and review of clinical policies and procedural guidelines.
- Take part in the provider on-call rotation as necessary.
Qualifications
- Board Certified Nurse Practitioner (ANCC or AANP) – [REQUIRED]
- Current, active Nurse Practitioner license in Massachusetts – [REQUIRED]
- DEA Registration to prescribe controlled substances – [REQUIRED]
- Certification in Controlled Substances (CDS) – [REQUIRED]
- BLS Certification – [REQUIRED]
- Relevant experience working with frail or elderly populations – [REQUIRED]
- Willingness and ability to travel within the designated service area – [REQUIRED]
Certification Requirements
- AANP
- ANCC
- Basic Life Support (BLS)
- Controlled Substance Registration (CSR/CDS)
- Drug Enforcement Administration (DEA) registration
State License Requirements
- Valid Nurse Practitioner license in Massachusetts
Additional Licensure Requirements
- Nurse Practitioner certification with prescriptive authority.
