Job summary

Legacy Care is seeking an experienced Nurse Practitioner to join our Charlottesville team on a full-time basis. You will provide primary and complex care in post-acute, long-term care and assisted living settings, conducting comprehensive assessments and developing individualized treatment plans.

Ideally you bring Virginia NP licensure, board certification and a DEA registration, plus a track record in geriatric or outpatient care.

Qualifications

  • Master's degree or Doctor of Nursing Practice (DNP) from an accredited NP program.
  • Virginia NP license with eligibility to practice in Virginia.
  • National board certification in NP specialty; prescriptive authority and DEA registration.
  • Experience in post-acute, long-term care, assisted living or related clinical settings preferred.
  • Strong clinical assessment, diagnostic reasoning, risk assessment and treatment planning skills.
  • Proficiency with EMR systems and timely, accurate documentation practices.
  • Excellent communication, collaboration, organisation and problem-solving abilities.
  • Ability to work independently while collaborating with physicians and multidisciplinary care teams.
  • Availability to provide reliable full-time NP coverage based on patient and facility needs.

Responsibilities

  • Provide comprehensive assessment, diagnosis and treatment to patients in post-acute, long-term care and assisted living settings.
  • Conduct initial, routine and follow-up evaluations, including review of medical history, symptoms, medications, diagnoses and current concerns.
  • Assess and manage a range of medical conditions within the scope of practice.
  • Evaluate vital signs and physical findings, risk factors, functional status and changes in clinical presentation.
  • Develop, implement and review individualized treatment plans in collaboration with patients, families, facility staff and interdisciplinary care teams.
  • Prescribe, review and manage medications in accordance with applicable regulations and patient needs.
  • Monitor treatment response, therapeutic outcomes, adverse effects, drug interactions and polypharmacy.
  • Manage changes in medical status, acute clinical needs and escalating care when required.
  • Order, review and follow up on labs and tests; collaborate with care teams.
  • Participate in care planning meetings, interdisciplinary reviews and facility clinical meetings.
  • Support transitions of care and coordination of services across the continuum.
  • Provide education and support to patients, families and facility staff regarding diagnoses and treatment.
  • Promote evidence-informed practice, patient safety, infection prevention and quality improvement.
  • Maintain accurate, timely documentation in the EMR.
  • Communicate effectively with supervising physicians and care team members.

Role Overview: Full-Time Nurse Practitioner, Charlottesville, VA

We are seeking an experienced and collaborative Nurse Practitioner to support our Charlottesville team on a full-time basis. This role provides high-quality, person-centered primary and complex care within post-acute, long-term care and assisted living settings. The successful candidate will conduct comprehensive patient assessments, develop and manage individualized treatment plans, monitor treatment effectiveness, coordinate care with interdisciplinary teams and promote safe, effective continuity of care.

Key Responsibilities
  • Provide comprehensive assessment, diagnosis and treatment to patients in post-acute, long-term care and assisted living settings
  • Conduct initial, routine and follow-up evaluations, including review of medical history, symptoms, medications, diagnoses and current concerns
  • Assess and manage a range of medical conditions within the scope of practice

  • Manage changes in medical status, acute clinical needs and urgent patient concerns, escalating care appropriately when required
  • Order, review and follow up on relevant laboratory tests, diagnostic investigations and other clinical information
  • Collaborate with physicians, nurse practitioners, nurses, specialists, hospitals and community healthcare resources
  • Participate in care planning meetings, interdisciplinary reviews, family consultations and facility clinical meetings as required
  • Support appropriate transitions of care, hospital follow-up and coordination of healthcare services across the continuum
  • Provide education and support to patients, families and facility staff regarding diagnoses, medications, treatment plans, symptom management and prevention of clinical deterioration
  • Promote evidence-informed practice, patient safety, infection prevention, risk management and quality improvement
  • Maintain accurate, timely and clinically appropriate documentation in the Electronic Medical Record (EMR)
  • Communicate effectively with the supervising or collaborating physician and other members of the care team, as applicable
Qualifications And Required Skills
  • Master's degree or Doctor of Nursing Practice (DNP) from an accredited nurse practitioner program
  • Current, unrestricted Virginia Nurse Practitioner license and eligibility to practice in Virginia
  • Current national board certification in the relevant nurse practitioner specialty
  • Current prescriptive authority and Drug Enforcement Administration (DEA) registration, as required
  • Experience in post-acute, long-term care, assisted living, geriatric care, outpatient care, primary care, hospital or related clinical settings preferred
  • Experience caring for older adults and patients with complex medical needs preferred
  • Strong clinical assessment, diagnostic reasoning, risk assessment and treatment planning skills
  • Knowledge of chronic disease management, geriatric care, dementia-related care considerations and care coordination
  • Proficiency with Electronic Medical Record systems and timely, accurate documentation practices
  • Excellent communication, collaboration, organisation and problem-solving abilities
  • Compassionate, culturally sensitive, person-centred approach to patient care
  • Ability to work independently while collaborating effectively with physicians and multidisciplinary care teams
  • Availability to provide reliable full-time Nurse Practitioner coverage based on patient and facility needs
