Overview

Join to apply for the Nurse Practitioner - Field role at Lightways Hospice and Serious Illness Care.

We are a nonprofit healthcare provider serving 11 counties in northwest Illinois, offering serious illness care, hospice care and grief support. We currently have a full-time Nurse Practitioner position to assist in caring for hospice field patients. The NP will conduct face-to-face visits, sign E-scripts, attend IDT meetings, and assist the team in caring for patient and family needs. The NP will travel to patients’ homes, nursing homes or long-term care facilities. This role works Monday–Friday, 8:00 am–4:30 pm, primarily serving the southern team with possible travel to other areas as deemed necessary.

The position is eligible for full benefits including medical, dental, vision, PTO and 401k.
