Job summary

Your Health in South Carolina is seeking a Nurse Practitioner to service patients across Hilton Head Island area. This full-time role covers visits in designated service zones, with travel between facilities and homes required on a daily basis.

A base salary plus monthly bonus is available, with shift options and a 12-hour on-calls/off pattern or a standard 8-hour weekday schedule. Applicants must have an NP license in good standing and be able to provide high-quality, patient-centered care

Qualifications

• Licensed NP/PA/MD with a valid license in the state of employment.
• Experience delivering patient care in home health, clinic, facility, telehealth, or specialty care is preferred.
• Strong communication and teamwork skills within an interdisciplinary group.
• Ability to evaluate high-risk patients and develop care plans.

Responsibilities

• Evaluate high-risk patients with complex medical needs and develop individualized care plans.
• Coordinate care activities with the interdisciplinary team to ensure continuity of care.
• Communicate regularly with case managers, clinical coordinators, and other team members.
• Participate in team meetings to discuss patient progress and care coordination.
• Foster collaboration to ensure seamless patient management across settings (homes, facilities, telehealth, clinics).
• Document care management activities accurately to support patient care plans and compliance.
• Provide patient education and resources related to conditions and preventive care.
• Support patients and families to promote active participation in care.
• Adhere to HIPAA, Medicare, and applicable healthcare regulations.
• Monitor quality measures and identify opportunities for improvement.

We are seeking qualified Nurse Practitioner to service patients in theHilton Head Island area.

Our Provider role performs visits in their designated service area. You must have reliable transportation as travel is required daily.

This is a full-time salary-based position, with the opportunity to receive a monthly bonus.

Your Health is currently looking for a Nurse Practitioner to join our growing primary care family. A successful Provider will be able to perform these essential duties and responsibilities. Areas Of Responsibility

• Evaluate high-risk patients with complex medical needs and develop individualized care plans.
• Coordinate care activities with other members of the interdisciplinary team to ensure continuity of care and optimal patient outcomes.
• Communicate regularly with case managers, clinical coordinators, social workers, specialists, pharmacists, health coaches, and other team members to coordinate patient care.
• Participate in interdisciplinary team meetings to discuss patient progress, treatment plans, and care coordination efforts.
• Foster effective communication and collaboration to facilitate seamless patient management and interdisciplinary teamwork.
• Meet staff efficiency expectations while providing high-quality care to patients in various settings, including homes, facilities, telehealth, clinics, and specialties.
• Document care management activities accurately and efficiently to support patient care plans and regulatory compliance.
• Provide patients with education and resources related to their conditions, treatment plans, self-management strategies, and preventive care measures.
• Offer support and guidance to patients and their families to enhance their understanding of healthcare options and promote active participation in their care.
• Adhere to all relevant healthcare regulations, policies, and procedures, including HIPAA and Medicare guidelines.
• Monitor quality measures and gaps in care, identifying opportunities for improvement and implementing corrective actions as needed

Qualifications

• Nurse Practitioner (NP), Physician Assistant (PA), or Physician with a valid license to practice in the state of employment required. License must be in good standing.
• Demonstrated experience in providing patient care, preferably in a home health, clinic, facility, telehealth, or specialty care setting.
• Strong communication, collaboration, and interpersonal skills, with the ability to work effectively within an interdisciplinary team.
• Ability to evaluate high-risk patients, develop care plans, and coordinate care activities to meet patient needs and improve outcomes.
• Commitment to delivering compassionate, patient-centered care and promoting a culture of excellence in healthcare delivery.
• Strong organizational and time management skills, with the ability to multitask and prioritize responsibilities effectively.
• Ability to read and communicate effectively.
