Matter Health in Detroit, MI is seeking a Nurse Practitioner to support older adults across public housing communities. You will serve as their ongoing primary care provider, building trusted relationships and delivering comprehensive, patient-centered care with clinical autonomy in a community-based model.

This role involves travel to homes and housing sites, collaboration with an interdisciplinary team and a Medical Director, after-hours on-call, and a mix of telehealth and in-person visits,

Qualifications

• Graduate degree from an accredited Nurse Practitioner program.
• Active, unrestricted Nurse Practitioner license in Michigan.
• Current national board certification (AANPCB or ANCC) with Family/Adult/Gerontology scope.
• Meet credentialing/enrollment/privileging requirements.
• 3+ years clinical NP experience, preferably with aging/medically complex populations.
• BLS certification.
• Valid driver's license and transportation (car stipend).
• Experience with EHR documentation (athenahealth preferred).
• Commitment to equitable, patient-centered care.

About the Role

Matter Health is seeking a Nurse Practitioner to support older adults in Detroit, MI.

At Matter Health, our NPs are more than just providers - they are Panel Owners. You will manage a dedicated panel of older adults as their ongoing primary care provider, building strong, lasting relationships within the communities you serve.

This is a fully community-based position with no traditional clinic home base. Nurse Practitioners regularly travel throughout their assigned market to provide care in affordable housing communities and patients' homes, building strong relationships and ensuring consistent access to care across their designated territory. Nurse Practitioners work with a high degree of clinical autonomy while collaborating closely with an interdisciplinary care team and a dedicated Medical Director for support in complex cases.

Key Responsibilities

Qualifications

• Graduate degree (Master's or Doctorate) from an accredited Nurse Practitioner program




• Active, unrestricted Nurse Practitioner license in Michigan




• Current national board certification as a Nurse Practitioner through an approved certifying body such as AANPCB or ANCC, with certification appropriate to the patient population served (Family, Adult, Gerontology, or Adult-Gerontology)




• Ability to meet applicable professional credentialing, enrollment, and privileging requirements




• 3+ years of clinical experience as a Nurse Practitioner, preferably working with aging or medically complex populations




• Basic Life Support (BLS) certification




• Valid driver's license and reliable transportation (car stipend provided)




• Experience with EHR documentation (athenahealth preferred)




• Commitment to equitable, patient-centered care

Strongly Preferred

• Active DEA registration and ability to meet applicable Michigan controlled substance prescribing requirements




• Background in family medicine, internal medicine, adult medicine, or geriatrics




• Experience in value-based care settings




• Experience providing care in community-based, in-home, or non-traditional healthcare environments




• Familiarity with MEAT documentation and chronic condition coding




• Telehealth experience

• Actual compensation may vary from posting based on geographic location, work experience, education and/or skill level
