The Inpatient Rehab Nurse Practitioner (NP) will provide comprehensive care to patients undergoing rehabilitation after a major illness or injury. The NP will collaborate with a team of healthcare professionals to assess, diagnose, and develop treatment plans aimed at optimizing recovery and enhancing the quality of life for patients in an inpatient rehab setting.

Key Responsibilities:

  • Patient Assessment:
  • Conduct thorough physical examinations and health assessments on patients admitted for rehabilitation.
  • Review patients medical history, medications, and rehabilitation progress.
  • Diagnosis and Treatment:
  • Develop and implement individualized treatment plans in collaboration with physicians and rehabilitation therapists.
  • Monitor patients progress and adjust treatment plans as needed.
  • Provide education to patients and families regarding rehabilitation processes, treatment goals, and home healthcare instructions.
  • Counsel patients on lifestyle modifications and self-management strategies post-discharge.
  • Collaboration and Coordination:
  • Work closely with a multidisciplinary team, including physicians, physical therapists, occupational therapists, and speech therapists, to enhance patient care.
  • Participate in interdisciplinary team meetings to discuss patient progress and discharge planning.
  • Documentation:
  • Maintain accurate and up-to-date patient records in compliance with regulations and facility policies.
