Job summary

Voices for Voices is seeking a Nurse Practitioner to deliver evidence-based care in two VA Community Based Outpatient Clinics, split between Meadville, PA (3 days) and Franklin, PA (2 days).

The NP will manage an adult patient panel with acute and chronic conditions, develop individualized care plans, order labs, prescribe treatments, ensure patient education, and collaborate with the interdisciplinary team to achieve optimal outcomes. Eligible for Education Debt Reduction Program.

Qualifications

• Licensed independent practitioner (NP) with advanced education and national certification.

Responsibilities

• Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice
• Develops and implements an individualized plan to achieve optimal population health and disease management
• Demonstrates leadership and works collaboratively with the interdisciplinary team
• Orders and interprets labs, tests, and diagnostic studies
• Prescribes pharmacologic and non-pharmacologic interventions
• Initiates referrals and consults to specialty services
• Provides education to patients and their families
• Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes

Job description

Summary

This position is eligible for the Education Debt Reduction Program (EDRP) - a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval - award amount (up to $200 -000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply.

Duties

• The NP is a licensed independent practitioner with advanced education and national certification who provides evidence-based care for a population of patients
• The position is located in two of the VA's Community Based Outpatient Clinics (CBOCs) and will be split between Meadville - PA 3x/week and Franklin - PA 2x/week
