Job summary

Jefferson University Physicians in Philadelphia seeks a Nurse Practitioner to perform comprehensive assessments, diagnose, treat, and manage care for acute and complex chronic patients. Qualifications

• Master’s Degree is required.
• National Board Certification as a Nurse Practitioner is required.
• BLS certification is required.

Responsibilities

• Performs complete history and physicals and documents in the medical record.
• Orders, collects data using assessment techniques and relevant diagnostic procedures.
• May serve as the first responder for nursing and house staff during patient status changes and interventions.
• Assesses risks in acute and complex chronically ill patients, including meds, immobility, nutrition, and fluid/electrolyte balance.
• Collaborates with the interdisciplinary health care team to diagnose acute and complex chronic conditions.
• Manages diagnostic tests through ordering, interpretation, performance, and supervision.
• Formulates differential diagnoses by priority and orders appropriate interventions.
• Identifies expected outcomes and develops a plan of care across the continuum.
• Documents progress notes, plans of care, discharge summaries, and related documentation timely.
• Maintains ongoing communication with the care team and with patients’ families to ensure continuity of care.
• Demonstrates professional practice, preceptor/mentoring, and education of graduate students; participates in research and evidence-based practice.
• Maintains CEUs and membership in a professional organization.
• Prescribes diagnostic strategies and therapeutic interventions; may perform advanced procedures within privileges.
