Job summary

Vitality NP Services is seeking a Nurse Practitioner to join our team in a Skilled Nursing Facility in Mount Laurel, NJ. This in-person role offers full- or part-time opportunities with flexible scheduling and no evenings, weekends, holidays, or on-call requirements.

You will perform comprehensive assessments, collaborate with PCPs and specialists, and contribute to a high-quality interdisciplinary care team while earning strong productivity-based compensation.

Qualifications

• RN experience required; NP graduates welcome to apply.
• Strong clinical assessment and decision-making skills.
• Ability to work in a fast-paced subacute/long-term care setting.
• Excellent written and verbal communication.
• Comfortable with electronic documentation and computer systems.

Responsibilities

• Perform comprehensive assessments for new admissions and acute issues in subacute and long-term care patients.
• Prevent avoidable hospital transfers through early evaluation and timely intervention.
• Collaborate with primary care providers and specialists.
• Participate as an active member of the interdisciplinary care team.

Key Responsibilities

Compensation

$120,000 – $170,000 per year
(Full Time Base Salary + Productivity Bonus)
$65.00-$75.00 per hour
