Job summary

The Judge Group is seeking a Nurse Practitioner for a Senior Community Care (SCC) program in the Hanover, PA area. The NP will participate in a longitudinal, integrated care delivery program coordinating clinical care in a facility setting.

Schedule is Mon-Fri, 8 am - 5 pm with no on-call, no weekends, and no holidays required. Responsibilities include comprehensive assessments, managing conditions, documentation with ICD-10, care planning conversations, and patient/caregiver education.

Qualifications

• Active board-certified NP license (FNP/ANP/GNP/AGACNP).
• Active DEA licensure/prescriptive authority.
• Full COVID-19 vaccination.

Responsibilities

• Conduct comprehensive assessments.
• Effectively manage acute and chronic medical and behavioral conditions.
• Ensure accurate and complete documentation, including ICD-10 diagnoses.
• Conduct advanced illness and advanced care planning conversations.
• Provide patients and caregivers with counseling and education.
