Schedule: Will include every other weekend with possible holiday rotations unless otherwise noted

The Nurse Practitioner – MC will provide high-quality, evidence-based primary and family care to a panel of patients in a retail clinical setting. This role works collaboratively with a dedicated clinical care team to drive positive patient outcomes through accurate assessment, diagnosis, treatment, health counseling, and care planning. The position reports directly to the Practice Manager and requires the ability to practice autonomously while supporting team-based care.
Provide primary and family evidence-based care, including assessment, diagnosis, treatment, management of health conditions, health counseling, and disposition planning
Manage patient appointments and perform routine physical examinations
Conduct diagnostic testing and coordinate care with other healthcare professionals
Educate patients on healthy lifestyle choices, chronic condition management, and preventive care
Encourage patient engagement through health screenings, wellness services, and behavioral health support
Perform administrative duties including opening and closing the clinic, inventory management, patient follow-up, insurance verification, payment collection, and maintaining a clean and safe clinical environment
Collaborate daily with a clinical care team to ensure quality outcomes across the patient panel
Master’s Degree from an accredited Family Nurse Practitioner program
Current National Board Certification
Active state license to practice as an Advanced Practice Nurse
Current BLS certification from the American Heart Association (AHA) or American Red Cross (ARC)
Active BLS must be provided upon offer acceptance
Verifiable High School Diploma or GED
Minimum of 1 year of recent primary care experience as a Nurse Practitioner in a family medicine or primary care setting
OR successful completion of primary care training program
Working knowledge of quality measurement in chronic disease management
Ability to work autonomously without direct supervision
Strong clinical judgment, initiative, and patient advocacy skills
Effective verbal, written, and electronic communication skills
Strong organizational skills and ability to multitask
Proficiency with healthcare information systems and technology
Ability to collaborate frequently with interdisciplinary healthcare professionals
Willingness to obtain multi-state licensure as needed
Ability to work within a remote clinical care team culture when applicable
Weekend and holiday rotations may be required
