Job summary

Hueman is actively seeking Nurse Practitioners to conduct in-home Health Risk Assessments for Medicare/Medicaid members in and around Snohomish County, Washington. The role offers a flexible schedule and travel to patient homes, enabling meaningful patient connections outside the clinic.

Qualified NPs will perform non-invasive assessments, review histories, and provide wellness education. A state NP license and board certification are required to practice in-home.

Qualifications

• Masters degree or commensurate experience with NP license.
• Active State NP license.
• Active national board certification.
• Current BLS certification.
• 12 months of experience as a paid advanced clinical provider preferred but not required.

Responsibilities

• Travel to patients' homes.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history and ensure documentation.
• Conduct medical reconciliation to ensure patient safety.
• Provide basic patient education on wellness and preventative healthcare measures.

Position Summary

Qualified Nurse Practitioners interested in this position will have the opportunity to interact with members in a more intimate setting. Unlike a typical clinic visit, this role allows NPs to connect with patients on a more personal and effective level. Nurse Practitioners performing HRAs are not required to treat or prescribe in the home.

Nurse Practitioner Responsibilities
