American Physiatry in Connecticut is seeking a compassionate Nurse Practitioner to join our care team in skilled nursing facilities, focusing on pain control, function, and rehabilitation. You will work under the supervision of a Physiatrist and coordinate with PT and other clinicians.

The ideal candidate holds CT NP license, masters degree, and has experience managing a caseload independently, with strong documentation practices and a commitment to high-quality patient care.

Qualifications

• Licensed Nurse Practitioner with a valid Connecticut certification.
• Experience working independently while effectively managing your own caseload.
• Submit accurate and thorough clinical documentation promptly, ensuring compliance with company policies and regulatory requirements.

Responsibilities

• Conduct thorough assessments of patients, identifying and managing medical conditions under the supervision of a Physiatrist.
• Develop and implement individualized care plans, focusing on function and rehabilitation to improve patients' quality of life.
• Collaborate with the healthcare team to ensure comprehensive care, including medication management and therapy recommendations.
• Educate patients and their families on treatment plans, health maintenance, and preventive care.
• Monitor and evaluate patient progress, adjusting care plans as necessary to achieve optimal outcomes.
