Job description

A Nurse Practitioner contributes to AHF by delivering comprehensive sexual health, HIV prevention, and STI care at one of our Wellness Centers. In addition to evaluating symptoms, interpreting labs, and treating STIs in a timely manner. The Nurse Practitioner plays a key role in PrEP care- including same day PrEP starts when appropriate- through risk assessment, patient education, prescribing, follow-up planning, and coordination with the Wellness team and AHF Pharmacy to support initiation and retention. The Nurse Practitioner also supports our Rapid ART (Rapid Start) model by initiating antiretroviral therapy (ART) the same day a client is newly diagnosed with HIV ( as clinically indicated), ordering baseline labs, and ensuring streamlined results review and follow up within 24-48 hours. The Nurse Practitioner coordinates a warm handoff and timely transition to HIV primary care at an AHF Healthcare Center (HCC) so clients remain safely bridged from the starter pack to their first comprehensive HIV care visit. You will be a passionate advocate for our top initiatives.

To be successful as a Nurse Practitioner it is crucial to ensure that the AHF Core Values and Mission stay top of mind with all that you do. At AHF we are nimble and able to adapt in a dynamic environment to assist in providing the best experience for our clients and workplace for our employees.

AHF has a collaborative organizational structure where staff are accountable to multiple leaders.

The Nurse Practitioner will work as part of a close-knit team to accommodate walk-in clients while ensuring high quality, efficient care. This includes collecting and reviewing medical history, performing focused exams during medical visits, ordering and interpreting diagnostic testing, prescribing medications and/or treatment, and referring to a physician or specialist for consultation when needed. The Nurse Practitioner collaborates closely with Wellness staff, Linkage, and AHF Pharmacy to remove barriers to care, support same-day PrEP starts and ongoing PrEP follow-up, and ensure timely Rapid ART linkage for clients newly diagnosed with HIV.

  • Same-day PrEP starts: assess HIV risk/PrEP eligibility, provide affirming counseling, prescribe PrEP, order and review required labs, and ensure each PrEP clients leaves with a follow-up plan
  • PrEP follow-ups & retention: conduct follow-up visits (lab monitoring, side effect management, adherence support), coordinate refills and partner with Wellness staff and AHF Pharmacy to remove barriers so clients stay on PrEP.
  • Rapid ART (Rapid Start): initiate ART for newly diagnosed clients, order baseline labs, provide results-based follow-up within 24-48 hours (as clinically indicated), and coordinate timely linkage to HCC for ongoing HIV care
  • STI evaluation, treatment & results management: assess symptoms and exposures, order and interpret testing, provide timely on-site treatment when indicated, and manage results review/follow-up in a high volume, walk in setting.
