Job summary

AxessPointe Community Health Centers is seeking a Nurse Practitioner or Physician Assistant to deliver holistic primary medical and behavioral health services to diverse patients in the northeast Ohio area.

The ideal candidate will hold MSN-NP/PA credentials with Ohio licensure and board certification, and will work under a physician collaborative agreement to provide comprehensive care and promote preventive health in a team-based setting.

Qualifications

  • Formal MSN-NP/PA training and certification in the state of Ohio to practice medicine.
  • Licensure in the state of Ohio and board certification in family medicine (ANCC/NCCPA or equivalent).
  • Knowledge of community health centers' structure and operations.

Responsibilities

  • Examine, diagnose and treat illnesses and injuries across the lifespan.
  • Conduct well-child, well-woman and well-man exams for patients.
  • Order labs and radiology, interpret results, and design care plans.
  • Prescribe medicines and treatments per scope of practice.
  • Promote health education and disease prevention for all patients.
  • Provide age-appropriate care and collaborate with a team.
  • Refer to specialists as needed and coordinate with care teams.
  • Document patient records per program standards.
  • Possibly instruct medical or nursing students.
  • Operate within Ohio law and authorized scope of practice.

NURSE PRACTITIONER
SUMMARY

As part of a collaborative agreement with a physician as mandated by Ohio law and under the direction of the AxessPointe Community Health Center, Inc. (API) Medical Director, the NP/PA will provide quality primary medical and behavioral health services to adults, children and families in a holistic manner within the northeast Ohio area.

DUTIES AND RESPONSIBILITIES
  • Examines, diagnoses, and treats illnesses and injuries of patients throughout the lifespan.
  • Provides well-child, well-woman and well-man health maintenance exams to new and established patients.
  • Orders appropriate screening and diagnostic lab and radiographic exams.
  • Interprets results of physical exams and screening/diagnostic tests to implement appropriate care plans for the patient.
  • Prescribes medicines, therapeutic agents and treatment modalities as required by patient needs and in accordance with licensed prescriptive authority.
  • Supports health promotion, health education, health empowerment and disease prevention interventions for all patients.
  • Provides age-appropriate care to all patients.
  • Promotes patient and family centered care through a collaborative team approach.
  • Participates in established call group as necessary.
  • Refers patients to specialty practices as appropriate and needed. Provides medical care to patient as part of mutual patient care with specialist.
  • Confers with collaborating physician about patient care as necessary.
  • Documents and maintains patient records of services provided according to program standards.
  • May instruct medical students or nursing students.
  • Operates within a scope of practice as agreed to by the FNP/PA, the collaborating physician, the medical director and in compliance with Ohio state law.
  • Supervision and Administrative experience helpful
  • You may be asked to perform any of the job duties within your respected scope of practice at any AxessPointe location.

Requirements

KNOWLEDGE, SKILLS AND ABILITIES
  • Knowledge of principles and practices of preventative medicine.
  • Knowledge of state and federal laws pertaining to medicine and public health.
  • Formal MSN-NP/ PA training and certification in the state of Ohio to practice medicine.
  • Knowledge of the structure and function of community health centers.
  • Skill in communicating effectively with patients and families to include culturally sensitive care.
  • Skill in establishing and maintaining effective working relationships with other employees, patients and the general public.
  • Commitment to AxessPointe mission, vision and values.
MINIMUM QUALIFICATIONS
  • Graduation from an accredited MSN/ARC-PA program
  • Bilingual a plus
LICENSES OR CERTIFICATIONS REQUIRED
  • Board certification from ANCC/NCCPA or other valid NP/PA accreditation body with a focus in family medicine (all ages)
  • Licensed in the state of Ohio
  • Current DEA Registration
COMPLIANCE

This position requires compliance with API's written standards, including its Compliance Program and Standards of Conduct and policies and procedures. Such compliance will be an element considered as part of the regular performance evaluation.

Failure to comply with API's Written Standards, which may include the failure to report any conduct or event that potentially violates legal or compliance requirements or API's Written Standards or, for managers and supervisors, fails to detect non-compliant conduct where reasonable efforts would have resulted in detection, will be met by the enforcement of disciplinary action, up to and including possible termination, in accordance with API's Compliance Program Policy and Procedure: Addressing Instances of Non-Compliance through Appropriate Disciplinary Actions.

The noise level in the work environment is low to moderate. Stress level can be high at times.
