Job summary

Qualifications

• Must hold a valid Nurse Practitioner license.

Responsibilities

• Design personalized care and wellness plans integrating nutrition and lifestyle changes.
• Order and interpret labs; document results in the EHR.
• Collaborate with physicians and wellness specialists to support patient goals.
• Provide ongoing follow-up and patient education to improve health outcomes.
• Contribute to population health and quality improvement initiatives.
