As a key member of our clinical team, you will play a crucial role in reducing hospitalizations and improving the lives of nursing home patients through remote monitoring, chronic care management, and psychiatric collaborative care.

Job Summary

As a Nurse Practitioner, you will be responsible for providing on-site visits to skilled nursing facilities, while also working remotely from home with the support of a dedicated scribe team for efficient documentation. Your success in this role depends on building strong relationships with the facility, maintaining a patient-centered approach, and being a collaborative team player.

Responsibilities

• Provide high-quality preventative care to residents in skilled nursing facilities

Requirements
