Job Summary:

As a Nurse Practitioner with our organization, you will have the opportunity to deliver high-quality preventative care to residents in skilled nursing facilities, while enjoying a hybrid work arrangement that combines on-site visits with remote work from home. Responsibilities:

• Provide preventative care to residents in skilled nursing facilities, including conducting on-site visits and remote monitoring of vital signs
• Collaborate with healthcare teams to develop and implement individualized care plans, while ensuring continuity of care and coordination with other healthcare providers
• Conduct documentation and charting in a timely and accurate manner, with the support of a dedicated scribe team
• Participate in multidisciplinary team meetings and clinical decision-making, utilizing your expertise in Geriatrics and Internal Medicine
