Overview

Sub‑Acute, Long‑Term Care is a sector of healthcare exploding with potential and work‑life balance flexibility (no on‑call or weekends). We are seeking a full‑time (40 hours per week) provider for our Casa Real located in Santa Fe, NM. This position can be filled by a nurse practitioner or physician assistant. New grads are welcome to apply. The base salary range is $115,000 – $125,000 per year, plus an incentive compensation bonus program. Responsibilities

The Advanced Practice Provider reports directly to the Director of Clinical Practice and/or the Area Clinical Practice Manager. The provider collaborates on‑site with the Center Leadership Team for center‑specific goals and priorities, and with the Medical Director and/or Attending Physician for patient‑specific medical care. The provider offers primary and urgent care services to nursing center residents in collaboration with other medical providers, and works collaboratively with Center Leadership to achieve AlignMed Partners’ mission of improving the lives we touch through high‑quality health care and everyday compassion.

Qualifications
  • Graduate from an accredited School of Nursing program, licensed as a registered nurse, and certified as a Nurse Practitioner with a Master’s Degree in Nursing and a concentration in Older Adult and/or Family Nurse Practitioner (FNP). Other concentrations or specialty tracks may be considered based on experience and business need.
  • Physician Assistant must be a graduate of an ARC‑PA accredited program, hold a PA National Certifying Exam (PANCE) credential, have state licensure, NCCPA certification, and an unrestricted professional license. DEA registration required.
  • Minimum of two years of experience working with adult or geriatric populations is preferred.
  • In states where practice is allowed, APRN with an unrestricted professional license and DEA; PA with an unrestricted professional license and DEA for each state in which they practice.

Posted Salary Range: $115,000 – $125,000 per year.
