Job summary

AACI Health Center seeks a dedicated Family Nurse Practitioner to join our multidisciplinary team in delivering high-quality primary care to underserved communities. This full-time role focuses on comprehensive assessments, care plans, and coordination with a diverse care team.

The position requires a CA NP license, BLS, DEA, and at least 2 years of primary care experience. We offer loan forgiveness programs, retirement plan, generous PTO, relocation support, and CME reimbursement.

Qualifications

• Master’s Degree or Doctorate from an accredited Nurse Practitioner program.
• California NP license (unrestricted).
• Board Certification from the California Board of Registered Nursing.
• Active and unrestricted DEA license.
• Current BLS certification.
• Minimum 2 years of Primary Care clinical work experience.
• Experience working in a community health center, FQHC, or underserved healthcare setting preferred.

Responsibilities

• Provide comprehensive primary care services, including assessment, screenings, diagnosis, treatment, and preventive health services.
• Conduct physical examinations, obtain medical histories, and develop appropriate treatment plans.
• Prescribe and manage medications, develop individualized treatment plans, and monitor patient outcomes in collaboration with the care team
• Educate patients on disease prevention, medication adherence, and overall wellness.
• Collaborate with physicians, behavioral health providers, and interdisciplinary teams to ensure coordinated, high-quality care
• Maintain accurate, timely, and complete documentation in the electronic health record (EHR).
• Coordinate referrals and consultations with specialists to support comprehensive patient care
• Support team-based operations, including meetings, cross-coverage for clinicians, and participation in after-hours on-call rotation
