Job summary

Self-Help for the Elderly seeks a Regular On-Call Nurse Practitioner to perform in-home hospice visits in San Francisco, supporting the hospice team in determining continued eligibility for hospice services.

You will assess patients, coordinate with physicians, document findings, and maintain compliant records while traveling as needed and upholding hospice principles.

Qualifications

• Active California NP license and current CPR certification.
• One year of professional nursing experience; home health, oncology or hospice preferred.
• Understanding hospice care principles and compliance with hospice requirements.
• Current CPR certification.
• Ability to travel via public transit or car; valid CA driver license.
• Background checks, TB clearance, and health screening required.
• Bilingual English and Cantonese preferred.

Responsibilities

• Assesses patient’s condition at recertification and as ordered by physician.
• Coordinates with attending physician, Hospice Medical Director, and hospice team to optimize care and resources.
• Obtains data on physical, psychological, social and spiritual factors influencing care.
• Provides detailed written narratives when life expectancy is six months or less.
• Maintains up-to-date patient records with plans, actions and goals.
• Maintains accurate and timely documentation of all care and ensures regulatory compliance.
• Performs other duties as assigned.

Reports To: Director of Patient Care Services

Summary:

Conducts face-to-face visits with hospice patients in their homes to assist the hospice team in determining whether the patient is still eligible for continuation of hospice services.

Essential Functions:

1. Assesses patient’s physical condition upon recertification and as needed in accordance with physician’s orders, plan of care and Self-Help Home Health & Hospice policies and procedures.

2. Initiates communication with attending physician, Hospice Medical Director, and hospice team members as needed to coordinate optimal care and use of resources for the patient and/or patient’s family.

3. Obtains data on physical, psychological, social and spiritual factors that may influence patient’s or family’s health status.

4. Provides a detailed written narrative when clinical findings support a life expectancy of six months or less.

5. Maintains up-to-date patient records so that problems, plans, actions and goals are accurately and clearly stated and changes are reflected as they occur.

6. Maintains accurate and timely documentation of all care delivered and ensures compliance with local, state, and federal statutes and regulations.

7. Performs other duties as assigned.

Qualifications:

1. Current California Licensed Nurse Practitioner.

2. One year of prior professional nursing experience. Home health, oncology or hospice experience preferred.

3. Must have understanding of hospice principles of care and philosophy and familiarity with hospice requirements.

4. Current CPR certification.

5. Must pass health screening for TB clearance, physical examination and background check.

6. Bilingual in English and Cantonese preferred.

7. Must be able to travel via public transportation or personal vehicle for performance of job duties. If driving, must have and maintain a valid CA driver license, satisfactory driving record, and auto insurance coverage as specified in Self-Help's policy.
