Job summary

Qualifications

• Requires physician assistant or nurse practitioner master's degree with current board certification (NCCPA/ANCC/AANP)

Responsibilities

• Delivers high-quality, cost-effective care incorporating guideline-directed medical therapy in a timely and equitable manner.
• Demonstrates advanced cardiac knowledge to care for patients in ambulatory setting.
• Implements primary and secondary prevention for cardiovascular disease and episodic care.
• Performs routine history and physical exams with interpretation of labs, EKGs, CXR, stress tests, and echocardiograms.
• Collaborates with the care team and participates in team-based care.
• Participates in billing/coding and uses EHR/LifeChart effectively.
• Educates and mentors APP students and new staff.
• Leads in development and evaluation of standards, policies, and procedures; completes safety trainings.
• Manages a daily patient panel and coordinates care across settings.

Job description

This role is located on 950 Warren Ave, East Providence RI* 4-10-hour shifts

The ambulatory advanced practice provider (APP) is responsible for providing patient- and family-centered care that is consistently high-quality and cost effective to ensure optimal care outcomes for patients with cardiac diseases. The APP reports directly to the Manager for APPs and the ambulatory Medical Director for the Division of Cardiology. The App manages a caseload of patients in partnership with the healthcare team to optimize health status and quality of life and coordinate care over time and across healthcare settings. The APP functions as a clinical leader, educator, consultant, and liaison to healthcare providers caring for the patients across the care continuum.

Principal Duties And Responsibilitiies

• Delivers high-quality, cost-effective care incorporating guideline-directed medical therapy (GDMT) in a timely, efficient, and equitable manner to facilitate optimal care outcomes for a patient population with chronic cardiac diseases.
• Demonstrates advanced cardiac knowledge and skills to care for patients in the ambulatory care setting.
• Implements primary and secondary prevention interventions for cardiovascular disease, management of chronic stable cardiac diseases, and episodic care as needed.
• Able to perform routine history and physical examinations, integrating laboratory data, EKG, CXR, stress testing, echocardiograms and other diagnostic tests when formulating a plan of care in partnership with other members of the care team, patients and families.
• Demonstrates interprofessional collaboration and participates in team-based care.
• Participates in procedures for billing and coding for medical services. Competent use of EHR and Lifechart.
• Participates in the continuous education and training, clinical research, and quality improvement initiatives.
• Participates in education, training and mentoring of APP students and new employees.
• Provides leadership in the development, validation, implementation, and evaluation of standards, policies, procedures, and critical paths to support the patient care delivery model. Annually completes mandatory safety, infection control, BLS, Prescriptive privileges per licensure.

Basic Knowledge

• Graduated for a Physician Assistant program with a master's degree and has current board certification from the NCCPA or graduated from a master's program for nurse practitioners and has current board certification from ANCC or AANP

Experience

Demonstrated knowledge and skills necessary to provide care to patients with consideration of aging processes, human development stages and cultural patterns in each step of the care process

Work Environment And Physical Requirements

Manage a daily patient panel of 14-16 patients during a 10-hour shift, providing care across a broad spectrum of cardiovascular needs, including new patient consultations, chronic disease management, post-hospitalization follow-up visits, and urgent same-day or next-day evaluations. Practice within a collaborative, team-based model alongside cardiologists to deliver comprehensive, evidence-based cardiovascular care. Protected administrative time is built into the daily schedule, allowing dedicated time for documentation, care coordination, patient communication, test result management, and other essential non-clinical responsibilities within scheduled work hours.

Pay Range

$127,504.00-$161,782.40
