Pride Health is hiring a Nurse Practitioner in Providence, RI! (Contract)

Travel between multiple Long Term Care Facilities located closely together in the East Providence area

Schedule: Monday-Friday (8am-5pm)

Contract: 7-8 month contract

Pay Rate: $66-69/hour

Specific Duties:

• Provide specialized care to high-risk health plan members
• Work as part of an interdisciplinary team
• Manage acute and chronic medical and behavioral conditions
• Communicate and collaborate with the care team
• Facilitate advanced illness and care planning discussions
• Offer counseling and education to patients and caregivers
• No on-call
• No holidays required

Must have qualifications/experience:

• NP Certified in one of the following (FNP, ANP, GNP)
• Access to transportation
• 1+ year of experience (LTC, SNF, Geriatric preferred)
