Job summary

Safe Harbor Recovery Center in Portsmouth, Virginia is seeking a Nurse Practitioner to provide general medical care under physician direction. The role includes history and physicals, medication management, and clinical assessments onsite with possible telehealth collaboration.

Requires MSN and RN experience with the target client population, plus experience prescribing psychotropic meds in primary care and strong crisis management skills.

Qualifications

• MSN degree from an accredited school of nursing.
• One or more years’ experience as an RN with the client population.
• Experience in prescribing basic psychotropic medications that would ordinarily be managed in a primary care setting.
• Clinical experience with limit setting, therapeutic boundaries, crisis management, environment, and client care.

Responsibilities

• Gathers, develops, and evaluates age-appropriate history and physical data for physician’s use.
• Assesses physical, psychosocial and cultural needs and plans care.
• Provides screening and medication management to clients.
• Establishes priorities in planning and organizing client care and Master Treatment Plan.
• Encourages family involvement and provides education related to medical needs.
• Diagnoses and treats acute, episodic, or chronic illness, independently or as part of a healthcare team.
• Assists the physician with the clinical operation of the program.
• Documents data and charts clinical observations and treatments per guidelines.
• Collaborates with physicians and staff on outcome goals.
• Implements health care interventions under FNP prescriptive practice.
• Reviews medical reports released under the facility’s name.
• Provides guidance to clinical staff to ensure quality care.
• Prepares discharge summaries for physician review.
• Under collaborative practice, calls permissible medications to pharmacies.
• Provides on-call responsibilities as scheduled.
• May order, perform or interpret diagnostic tests and prescribe medications.

About the Job:

Purpose Statement:

The Advanced Registered Nurse Practitioner is responsible for providing general medical care and treatment to clients, under the direction of physician. Performs history, physical examinations, and preventive health measures on assigned clients; provides consultation and liaison throughout the facility; performs daily rounds on clients as directed by supervising physician; prescribes medications; and provides clinical assessments and diagnosis. Works collaboratively with the medical/clinical staff under the guidelines of state licensure for Nurse Practitioners. This position is onsite at the facility but may include some telehealth work to be determined by mutual agreement with the CEO.

Roles and Responsibilities:

Essential Functions:

• Gathers, develops, and evaluates age-appropriate history and physical (H&P) client data in collaboration with client, family and health care team for physician’s use in treating clients.
• Assesses and reassess physical, psychosocial and cultural needs of clients and formulates measures to meet client needs.
• Provides screening and medication management to clients served.
• Establishes priorities in planning and organizing client care based on H&P assessment. Participates in the formulation of client Master Treatment Plan
• Encourages family involvement in client care. Responds to identified client and family educational needs. Assists in client teaching as it relates to managing medical needs or medication education.
• Diagnoses and treats acute, episodic, or chronic illness, independently or as part of a healthcare team.
• Assists the physician with the clinical operation of the program
• Documents pertinent data including assessment, interventions, and evaluations. Charts clinical observation (direct or through dictation), evaluations and treatment of clients in required time according to facility guidelines.
• Collaborates with physicians and other staff on establishing realistic outcome goals as needed.
• Implements appropriate health care interventions as needed, using appropriate guidelines of Family Nurse Practitioner (FNP) prescriptive practice.
• Reviews medical reports released under the name of the Facility for assigned clients.
• Provides guidance to the clinical staff working with assigned clients to ensure that quality of care is provided.
• Prepares discharge summaries for physician review.
• Under physician collaborative practice agreement as a Family Nurse Practitioner with prescriptive privileges, will call permissible medication to pharmacies.
• Provides on-call responsibilities on scheduled basis directed by medical staff.
• Focuses on health promotion and disease prevention. May order, perform or interpret diagnostic tests such as lab work and x-rays. May prescribe medication.

Education/Experience/Skill Requirements:

• Master of Science of Nursing degree from an accredited school of nursing required.
• One or more years’ experience as an RN with the client population of the facility line of business required.
• Experience in prescribing basic psychotropic medications that would ordinarily be managed in a primary care setting.
• Clinical experience working with the client population of the facility; ability to demonstrate knowledge of concepts of limit setting, therapeutic boundaries, crisis management, therapeutic treatment, environment, and client care required.

Licenses/Designations/Certifications:

• Current Nurse Practitioner license required, per state regulations/requirements, may also require a current DEA license.
• CPR certification and de-escalation training required (training available upon hire and offered by facility).
• First aid may be required based on state or facility requirements.
