Job summary

Post Acute in Portland, OR, seeks a Nurse Practitioner to provide care under physician supervision in an in-patient Post Acute Long Term Care setting. You will examine, diagnose, treat illnesses, and write prescriptions within your scope while coordinating with the medical director and care team.

You will perform stabilizing visits, order labs, and collaborate to develop comprehensive care plans. Strong communication and geriatric care skills are essential to support residents and patients.

Qualifications

  • Successful completion of an approved nurse practitioner program with appropriate certification.
  • Current approval by the appropriate Board of Licensing.
  • Must be licensed (preferred), or eligible to obtain an unrestricted license as a nurse practitioner in the state.

Responsibilities

  • Meets with new resident/patient to perform initial stabilizing visit and manages patients with the attending physician.
  • Orders laboratory testing, diagnosis, and treatment of chronic and acute illnesses and minor injuries; writes prescriptions.
  • Performs clinical examinations.
  • Acts as a resource for decisions, planning, and policies affecting patient health.
  • Maintains inpatient records and compiles reports as needed for evaluations.
  • Collaborates with staff to identify patient needs and develop care plans.
  • Follows patient in the skilled nursing facility for continuity of care.
  • Provides general health education on diet, emotional well-being, and health maintenance.
  • Assists physician with emergency treatments and more complex procedures.
  • Serves on committees dealing with health care (Antibiotic Stewardship, etc.).

Job description

The Nurse Practitioner will provide healthcare services under the direction of a physician. The Nurse Practitioner will plan, direct and evaluate resident/patient care in an in-patient Post Acute Long Term Care setting; examine and treat chronic and acute episodic illnesses, minor injuries, accidents, and other injuries/illnesses; performs selected laboratory testing; use of clinically appropriate referrals to physicians, write prescriptions, diagnose and treat medical conditions within the scope of license expertise, protocols, and available equipment. The Nurse Practitioner will also work with the medical director in the care and treatment of residents/patients.

Job Duties/Responsibilities
  • Meets with new resident/patient to perform initial stabilizing visit/exam and medically manages patients with the attending physician.
  • Orders when medically necessary, laboratory testing, diagnosis, and treatment of chronic and acute episodic illnesses and minor injuries. Writes prescriptions within the scope of license, expertise, protocols, and available equipment. Schedules follow-up visits for self and the attending physician as needed.
  • Performs clinical examinations.
  • Acts as a resource person for decisions, planning, and policies affecting the health of resident/patient.
  • Maintains inpatient records. Compiles reports as needed for evaluations, etc.
  • Collaborates with staff to identify resident/patient needs and to help develop plan to meet these needs.
  • Follows resident/patient in the skilled nursing facility as needed for continuity of care.
  • Provides general health education regarding matters such as proper diet, emotional problems of daily living, and health maintenance.
  • Assists physician with emergency treatments and more complex procedures.
  • Serves as a member of committees dealing with health care (Antibiotic Stewardship, etc.)

Education and Experie nce

Physical Requirements
  • Able to lift 35 p ounds for routine performance of essential functions.
  • May be exposed to adverse indoor and outdoor environmental conditions, bloodborne pathogens, physical hazards (e.g., needle pricks, etc.) and chemical hazards.
  • Be able to perform prolong sitting, standing and walking greater than 15 minutes at a time.
